Docopa 200 mg Tablet contains Doxophylline, a bronchodilator used to treat respiratory conditions such as bronchial asthma, bronchospasm, and chronic obstructive pulmonary disease (COPD).
Pharmacology: Doxophylline works by selectively inhibiting phosphodiesterase 4, leading to relaxation of bronchial smooth muscle. Unlike Theophylline, it has a lower affinity for adenosine receptors, which may contribute to a better safety profile. Additionally, Doxophylline inhibits platelet-activating factor (PAF) and reduces leukotriene production.
Dosage and Administration:
- Adults: 400 mg two or three times daily, or as prescribed by a physician.
- Elderly: 200 mg two or three times daily.
- Children over 12 years: 200 mg two or three times daily.
- Children aged 6-12 years: 6-9 mg/kg body weight twice daily.
For example, a child weighing 10 kg would receive 3 ml (60 mg) twice daily.
Contraindications: Docopa is contraindicated in individuals with:
- Hypersensitivity to Doxophylline or other xanthine derivatives.
- Acute myocardial infarction.
- Hypotension.
- Lactating women.
Side Effects: Possible side effects include nausea, vomiting, headache, and gastrointestinal discomfort. In rare cases, severe cardiac arrhythmias and seizures may occur, especially in cases of overdose.
Precautions: Caution is advised when administering Docopa to patients with:
- Liver disease.
- Congestive heart failure.
- Chronic obstructive pulmonary disease.
- Severe hypoxemia.
- Hyperthyroidism.
- Peptic ulcer.
- Renal impairment.
Monitoring of blood levels may be necessary in these cases.
Drug Interactions: Docopa should not be used with other xanthine derivatives. Concomitant use with certain antibiotics (e.g., erythromycin), allopurinol, cimetidine, and propranolol may decrease the clearance of Doxophylline, leading to increased blood levels. Conversely, anticonvulsants like phenytoin and smoking may increase its clearance, necessitating dosage adjustments.
Use in Pregnancy and Lactation: While animal studies have not shown fetal harm, there is limited data in humans. Therefore, Docopa should only be used during pregnancy if clearly needed. It is contraindicated during lactation.
